The content of your file will be destroyed after 10 years unless there are legal reasons to retain a file. If any person or family member wishes to obtain personal items from a file before it is destroyed they should contact the Enable on 08 8419 5330. The information you give Enable with your consent is the only information we collect and hold. Enable will usually know your name, gender, contact details, some medical history, your symptoms, ethnic background and, if relevant to your health. We sometimes collect personal information from a third party if you have consented or would reasonably expect us to collect the information in this way, for example from a healthcare service such as a residential care facility who is managing your care.
It is your right to request access to information we hold about you at any time. Your personal information is information about you which can identify you and includes your name, address, date of birth, and can include information about your illnesses, symptoms, disabilities and any information collected about you to provide a health service. In dealing with personal information, Enable abides by 10 National Privacy Principles in the Privacy Act 1988 (Cth) and any applicable State or Territory privacy laws.
